Primary.Health discloses Personal Information to government entities, including law enforcement, only in the following circumstances: (1) when required to do so as a matter of law; (2) to assist in the investigation of a potential crime impacting Primary.Health, its employees, its customers, or the communities we serve; or (3) when required in response to legal process (e.g., subpoena, warrant).

In the past twelve months, we have collected, used, and disclosed personal information about you for the above purposes.
